Ишемическая болезнь сердца у лиц молодого возраста: особенности этиологии, клинических проявлений и прогноза

Аннотация

У больных молодого возраста с ишемической болезнью сердца (ИБС), помимо традиционных факторов риска развития сердечно-сосудистых заболеваний, важную роль играет ряд других предрасполагающих факторов, в том числе генетических. Молекулярно-генетическая диагностика используется для обнаружения моногенных заболеваний с высоким риском развития ИБС, таких как семейная гиперхолестеринемия. Для уточнения риска развития ИБС у больных без симптомов заболевания осуществляются попытки учета совокупности однонуклеотидных полиморфизмов, объединенных в шкалы генетического риска. У больных молодого возраста первым проявлением ИБС чаще становится острый коронарный синдром, чаще обнаруживают неизмененные или малоизмененные коронарные артерии и однососудистые поражения, а примерно в 20% случаев причина ИБС не связана с атеросклерозом. В целом у молодых больных ИБС прогноз лучше, чем у пожилых. При этом существуют гендерные различия в исходах ИБС у больных молодого возраста, причина которых не выяснена.
