Динамика распространенности хронической сердечной недостаточности и сахарного диабета в Нижегородской области с 2000 по 2017 год
Аннотация
Цель. Установить динамику распространенности, заболеваемости и общей смертности среди больных ХСН в зависимости от наличия СД в репрезентативной выборке Нижегородской области. Материалы и методы. В 1998 г. создана репрезентативная выборка Нижегородской области, включено 1922 респондента. С 2000 по 2017 гг. выборка обследована трижды. Анализировались больные ХСН и СД. Результаты. В течение 17 лет распространенность ХСН I-IV ФК (NYHA) выросла с 6,88 до 9,1% случаев (р=0,04). Распространенность ХСН III-IV ФК NYHA увеличивается значительно - с 1,2 до 4,8 % (р<0,001). Заболеваемость выросла среди больных ХСН от 10,8 до 16,4 на 1000 населения (р=0,02), а среди больных СД - с 3,7 до 6,6 на 1000 населения (р=0,03). СД значительно ухудшает прогноз жизни, особенно больных ХСН III-IV ФК NYHA. Заключение. Распространенность ХСН увеличивается. Заболеваемость и общая смертность зависят от наличия СД и тяжести ФК ХСН. После декомпенсации ХСН риски общей смерти увеличиваются в 5,9 раза независимо от наличия СД.
