Актуальные вопросы улучшения прогноза у пациентов с фибрилляцией предсердий, перенесших ишемический инсульт

В настоящее время доказавшим свою эффективность и постулируемым в международных клинических рекомендациях подходом к профилактике повторных инсультов у пациентов с фибрилляцией предсердий (ФП) является назначение антикоагулянтной терапии, причем все приоритеты отданы прямым пероральным антикоагулянтам. Обширная доказательная база свидетельствует о преимуществах ривароксабана перед прочими представителями класса в вопросе вторичной профилактики инсульта при ФП, причем это сочетается с оптимальным профилем безопасности. Терапия ривароксабаном может обеспечить наиболее благоприятный прогноз за счет профилактики повторного инсульта при ФП, снижения темпов прогрессирования поражения почек и замедления скорости атеросклеротического поражения сосудистого русла. Важным преимуществом ривароксабана является однократный прием препарата в сутки, что важно в контексте частого наличия когнитивных нарушений у больных этой категории, и может позволить улучшить приверженность, а значит и достичь ожидаемого профиля эффективности терапии. Таким образом, ривароксабан может рассматриваться как препарат выбора для вторичной профилактики инсульта при ФП.
